ITKT for Medical Emergencies in Indonesia

Foreign nationals in Indonesia sometimes face unexpected medical situations that make it impossible to leave the country before their visa or stay permit expires.

This often happens after surgery, hospitalization, or serious illness when a doctor advises the patient not to fly.

In situations like this, Indonesia provides a legal solution called ITKT or Izin Tinggal Keadaan Terpaksa, also known as an Emergency Stay Permit or Stay Permit Under Force Majeure. ITKT can help foreigners remain in Indonesia legally while recovering from a medical condition.

 

Example Case

A foreign traveler enters Indonesia on a Visa on Arrival (VOA) and has already extended the stay once. Note that a VOA can only be extended one time for 30 days.

  • Current stay permit valid until 2 June (and now it’s May 22nd)
  • The traveler recently underwent surgery
  • The doctor states the patient is not fit to fly yet
  • The traveler cannot leave Indonesia before the visa expires

In this situation, ITKT may become the appropriate legal solution to avoid overstay while the traveler is still under medical care.

 

What Is ITKT?

ITKT stands for Izin Tinggal Keadaan Terpaksa, a special emergency stay permit granted by Indonesian Immigration for foreigners who cannot leave Indonesia due to exceptional circumstances.

Medical emergencies are considered one of the humanitarian reasons that may support an ITKT application.

Unlike a regular visa extension, ITKT is intended for urgent and unavoidable situations where the foreign national is genuinely unable to travel.

ITKT can generally provide an additional stay period of up to 30 days.

 

Do Not Wait Until the Last Minute

One of the biggest mistakes is waiting too close to the visa expiry date.

Immigration officers usually need time to review documents and assess the emergency condition. Starting the process early gives better chances for smoother handling.

As a practical guideline:

  • If the doctor already confirms the patient should not travel, prepare documents immediately
  • Ideally start discussions several days before the current permit expires
  • Do not assume overstay will automatically be forgiven

Medical Documents Are Very Important

Immigration needs clear supporting evidence showing the foreign national is medically unable to travel.

The documents should be detailed, recent, and consistent.

Required Documents

  • Passport
  • Current visa
  • ITK or stay permit details
  • Statement letter from hospital or doctor
  • Medical records
  • Photos showing the foreign national receiving treatment or hospitalization

The doctor or hospital letter should ideally explain:

  • The medical condition
  • Why the patient cannot travel
  • Estimated recovery period
  • Estimated date when the patient may be fit to fly again

The clearer the explanation, the easier Immigration can understand the emergency situation.

 

Does the Applicant Need to Visit Immigration?

In most cases, yes. Foreign nationals applying for ITKT are generally still required to report to the Immigration Office for processing and verification.

However, for severe medical conditions or hospitalization, Immigration officers may conduct a visit to the applicant’s location instead of requiring the applicant to appear in person.
